What information do I need to bring with me to the Hospital

Patients should bring the following

  • Insurance Card / Company Referral Letter
  • Your Company ID Card
  • CNIC & Copy

Referral forms from your physician (if required by your insurance carrier)

What is the admission & Discharge Process

For admission you need prescription from consultant and provide Copy of CNIC to our reception. They will prepare your file which carried Admission form and information checklist. At the time of admission they will hand over 2 Attendant passes and brief you accordingly.

For Discharge the concern department will prepare discharge summary after getting order from consultant and will be sent to billing department for clearance.

Does Hospital have diagnostic services & Support of pharmacy

Yes we have State of the art Pathology & Radiology departments running under the supervision of highly qualified and well trained staff. Our all procedures and reports are reliable.

Our pharmacy is fully operational for 24 hrs.

Where do I go to donate blood (either for my forthcoming operation or for a sick relative)

Call the local Red Cross at ___________ & ____________ to arrange for such a donation. Any donations can be designated for you or for the Hospital at the time of giving.

Does Hospital provide 24hrs coverage to patients

May I take flowers or gifts to a patient at HLH

Gifts are allowed on all units. Flowers are allowed on all units except the Intensive Care Unit.

What are the patient visiting hours

Our Visitors hrs as: 9am to 12pm and 7pm to 10pm.

Are children allowed to visit patients at HLH

In order to provide a more restful environment for our patients, on most of the nursing units, children under 10 years of age may visit for only 15 minutes. In the Maternal Child Unit no visitors under 12 years of age are permitted except siblings of the newborn. In the Pediatrics Unit, no visitors under the age of 18 months are permitted and must be accompanied by a parent.

Why can’t children other than siblings of the newborn visit the Maternal Child Unit

The Maternal Child Unit, where new mothers and their babies are cared for, has strict visitor regulations to protect the health of both the newborn infants and any youngsters less than 12 years of age, who are more vulnerable to infectious disease.

Why do I need to have a Visitor/ Attendant Pass

We require everyone entering the Hospital to identify themselves and their destination within the hospital to provide a safer environment for our patients, visitors, Attendant and staff.

How can I contact a patient in the Intensive Care Unit (ICU)

Intensive Care Unit patients have no phones in their rooms. Intensive Care Unit nurses will relay information to immediate family members only.

Can I take a bus to the Hospital

We are located right in the center of city on main Ferozpur road Opposite “Gaddafi Cricket Stadium” 14- Abu Baker Block New Garden Town, Lahore.

If you are traveling through “Metro Bus Service” then The Station Number 14 Gaddafi Stadium is just front of us.

What is the Medical Records Department

This is the new name hospitals use to designate the department that holds your medical records.

How can I get a copy of my medical records for myself or to be sent to someone other than myself, such as a physician, attorney, insurance company or any other person

To obtain copies of your medical records, download and complete an Application for Information  form. Clearly specify the dates of service and/or the types of reports needed and also send authority letter along copy of CNIC.

How can I get a copy of my child’s birth certificate

All birth certificates are issued by the Medical Record Department From Monday to Saturday 08am to 04pm only to immediate family. For this purpose you need to fill Birth Certificate Performa.

Note: No information will share on phone call. Nominal fee charge for copy and all record provided in 24 hours

While I am waiting for the birth certificate to be processed by vital records, how can I obtain a verification of birth letter for my insurance

A birth verification form is provided complimentary at the time of discharge. If additional copies are needed, please contact the Medical Record Department.

Can I get my lab results over the phone

The physician can release the lab results over the phone if the results are being made available as part of a visit or treatment episode. All other requests for lab record should be directed to the H.I.M. Department.

How do I obtain a copy of my Radiology Films

To obtain films created after 7/24/04:

Contact the Film Library at 410-740-7924. The Film Librarian will require your name and date of birth to confirm your identity and locate your exams within the hospital computer system. Please be prepared with the date(s) and exams(s) for which you are requesting copies. HCGH digitally storages most radiology images so your images can be provided to you on CD relatively quickly. You will be asked for a date/time you wish to pick up your films or CD.  Proper identification must be presented at time of pick-up. Upon arrival at the hospital, proceed to “Information” and ask to be directed to the Film Library. It is a short distance from the main entrance of the hospital and is open Monday-Friday from 6:00 a.m. to 6:00 p.m.

To obtain films (originals) created before 7/24/04:

Contact the Film Library as listed above. Films created prior to 7/24/04 are not stored digitally and will need to be returned to the hospital. However, if you are moving out of state, your films will be provided to you without the requirement that they be returned. The Film Librarian will determine if your films are stored locally or off-site. If the film are stored locally, you may pick them up at a mutually agreed upon date/time. If they are off-site, HCGH will require two (2) days to retrieve the films. Upon arrival of your films, you will be contacted and a date/time of pickup will be arranged. Proper identification must be presented at time of pick-up. Upon arrival at the hospital, proceed to “Information” and ask to be directed to the Film Library. It is a short distance from the main entrance of the hospital and is open Monday-Friday from 6:00 a.m. to 6:00 p.m.
